Is this correct? Mailing Address Format?

I am not sure if I am missing something or if my order of numbers is wrong. I am mailing a package from the states. A simple yes or no will be fine.

Name (first Last)

Street house number

city/county code city name, Germany

for example:

Black Smith

Huisnumer 6

01234 Example Germany

Please help me
