Is this enough to get my social security card replaced if I'm 17?

I completed the application and I have my birth certificate. I'm going to mail them in on Monday. Is this enough to get a new one?

  2. No, a birth certificate does not prove identity, only birth date.  You need to send in a legal photo ID or have your parent do the application and send an ID.

  3. It should be, but you can give your local Social Security office a call to see.  Actually, you probably don't need the card as long as you know the number.

  6. First of all, since you aren't an adult,  your parent has to be the one to request the replacement card.

    I have attached the requirements from Social Security Online.

    You need your original birth certificate, and proof of identity - the types accepted are listed; they include school records.  Your parent must request the card, and provide identification for themselves as well, such as a current drivers' license.

    These must be taken to the local social security office, not mailed.

    Replacement Social Security Number Card for a U.S.-born Child

    You can replace your child’s Social Security card for free if it is lost or stolen. You are limited to three replacement cards in a year and 10 during a lifetime. Legal name changes and other exceptions do not count toward these limits. For example, changes in immigration status that require card updates may not count toward these limits. Also, you may not be affected by these limits if  you can prove you need the card to prevent a significant hardship.  

    To get a replacement card for your child, you must:

        * Complete an Application For A Social Security Card (Form SS-5); and

        * Show us documents proving your child’s:

              o U.S. citizenship (if your child has not already established citizenship with us); and

              o Identity.

        * Show us documents proving your identity.

        * Take your completed application and original documents to your local Social Security office .

    Citizenship

    We can accept only certain documents as proof of U.S. citizenship. These include a U.S. birth certificate, U.S. consular report of birth, U.S. passport, Certificate of Naturalization or Certificate of Citizenship.

    Identity

    Your child: We can accept only certain documents as proof of your child’s identity. An acceptable document must be current (not expired) and show your child’s name, identifying information and preferably a recent photograph. We generally can accept a non-photo identity document if it has enough information to identify the child (such as the child’s name and age, date of birth or parents’ names). We prefer to see the child’s U.S. passport. If that document is not available, we may accept the child’s:

        * Adoption decree;

        * Doctor, clinic or hospital record;

        * Religious record (e.g., baptismal record);

        * Daycare center or school record; or

        * School identification card.

    You: We also must see proof of your identity. An acceptable document must be current (not expired) and show your name, identifying information (date of birth or age) and preferably a recent photograph. For example, as proof of identity Social Security must see your:

        * U.S. driver’s license;

        * State-issued nondriver identification card; or

        * U.S. passport.

    If you do not have one of these specific documents or you cannot get a replacement for one of them within 10 days, we will ask to see other documents, including:

        * Employee ID card;

        * School ID card;

        * Health insurance card (not a Medicare card);

        * U.S. military ID card; or

        * Adoption decree.

    We will mail your child’s card as soon as we have all of your child’s information and have verified your documents. Your child’s replacement card will have the same name and number as the previous card.

    Keep your child's Social Security card in a safe place. It is an important document. Do not carry it with you

  7. There should be directions on the application, but I don't think I'd give up my birth cirtificate.  Do you not have a SS office in your community?  It would be much easier to just go in!

  9. Usually u need 2 forms of IDs and u definely need to have a social security card to get a job or file taxes, etc.
