The jewels were put into settings or in the case of pearls you can drill a hole into them like a bead and were then sewn onto clothing.

Women did it as a sign of elegance. So they could litterally be wearing their wealth. Think of it today with people like p-diddy wearing diamonds on his watch and belts.
