My son is flying to another state with our best friend. I am wondering if I need to write some kind of letter stating that my son is with this person and that I know about it or something like that. He is 11 and I was planning on writing a letter in case he needs medical treatment. Also should it be notarized?

Thanks.

  1. Yes, you'll need a letter and it needs to be notarized.  The letter can be very basic but should include your name, his name, the name of the person he is traveling with, that they have your permission to seek medical treatment and your contact info.  Here's an example:

I, Your Name, give my permission to Friend's Name to take my son, Son's Name, to Destination.  In the event of an emergency, Friend's Name has my permission to seek medical attention for Son's Name.

    Signature

    Phone Number/Address

You should also give your Friend a list of any and all medical conditions your son has, any medication he's on along with doses, and the name and contact info of your son's pediatrician.  A copy of both the front and back of your health insurance card is a good idea as well.

  2. u can get a letter issued stating that XX would be accompnaying the kid.  the signatuare can be accompanied by a photocopy of an id of yours where there is a signature - eg passport

    AM

  3. A notary only guarantees that the signature is from the person that it stated.. he/she doesn't write anything.

A parent need to write a statement that they are allowing the child to travel with the friends parents (must be named and must be adults). This must be signed in front of a notary .

You will need to be specific and include medical treatment.

    Most airlines require this document,

  4. Yup you have to have to get a notarized letter of consent from a notary public. A notary public is a like a lawyer but cheaper.Make sure your passports are valid. Make sure the notarized letter also includes medical treatment

PS im not sure if have to get that since your only traveling inside your country, but i would do that just incase

PSS do not write it yourself, the travel ppl dont give **** if you write it yourself they do care if its from notary public
