Keith answered my question about the mircosoft excel and im not sure where to contact him at keith the form im looking to make deals with an address book in excel and this form somes up with last name first name , address ect for adding the information in the data base

Okay. To my knowledge this could either be a proprietary program, in which case the solution might be too large for this forum, or using Excel's built-in Form function. That is located under Data->Form in the menus, and is handy to enter information like that.

The only real requirement I can think of is to be sure that your columns have good headers and that they are all there, data is clean, no blank rows, stuff like that. Excel expects things to be very tidy for functions like this one to work.
