    I thinkyou may not have the right generalization.

    I would say rather that [kyaa] and other question words can be attached to the clause, or to the verb phrase, on the left or right periphery.

    The difference in scope of the Q-word accounts for the meaning contrasts to which you refer.

    E.g.:

    tum kyaa [kaam karte ho]?

    What work do you do?

    kyaa [tum kaam karte ho]?

    Do you work?

    Notice the contrasts in the position of [kyaa], the parsing, and the corresponding readings.

    if it is the only word in sentence there are two possiblities..

    1) kya???-- what?? i did not get you...

  5. Hindisikhnewaalaa is right when he says that even in second place kya could denote a question.

    Further,  if placed first,  it could still make a statement (albeit a provocative one meriting further questioning)  

    e.g. Kya kuchch nahin maine saha is zindagi mein!  (What all haven't I suffered in this life!)  Or  

    Kya bataoon wahan kya hua! (What do I tell about all that  happened there!)  

    More for Dramatic effect!
