On Monday( august 11th) I'm going to buffalo with my friend and her mom what does my mom gotta sign for me to go? i Iive in Toronto..( were going just for a day..)

Can anyone give me a sample piece of writing for me to go? I've got what is below but I'm not sure if i need more.. and do i have to put my complete full name? or will just the name i go by and first last name work?

I __________give my daughter _________permission to go with __________ to Buffalo,New York for the day,Monday,August 11th 2008,If a problem is to arise I give permission for __________to take action if an emergency includes my daughter.

What you have is all you need for the permission slip, along with the Pass port and health card be sure you have another picture ID.~
