For the Devils, I know you get refunded.  My friend's family bought game 5 tickets in 2006, but the Devils swept the Rangers :-) and they got their money back.

It might be different for different teams.  So if you're curious you can call a ticket representative for your team.

Sort of.

If you're a season ticket holder, most teams will simply credit your account the cost of the ticket rather than have to take the tickets back to the box office (or mail them back).

If not, you return them to the point of purchase for a refund.  However, in most cases, any processing fees, handling fees, Ticketmaster Gouging Fees, etc. may not be refundable (you get the price on the ticket); to that, if you bought the ticket through a third party and paid greater than face value, anything beyond the cost of the ticket...outta luck.

well it depends, my family owns season tickets and my dad baught playoff tickets including game 7 for the blackhawks (who didnt even make it to the playoffs). All that money just went to our account for next season, i guess it depends on who and where you buy it from.

Yes.  I bought tickets to the 2nd round a few years back and my team ended up getting knocked out in the 1st round.  I lost 2 dollars for a processing fee, thats it.
