Letter Format?

When we need to address people in a letter for example Mr Patrick Wong Hong Sun, do we start as

Dear Mr Wong Hong Sun, Patrick

and if the people has already married do we still address her as Ms Lai for example

Dear Ms Lai.

Please do advice.

For the first one you would  put:

Dear Mr Patrick Wong Hong Sun,

In letters you don't need to put the last name first, that is usually reserved for lists of people and other similar things.  For the second one you could either use Ms. or Mrs.  

Mrs. is used for all married women, but Ms. can be used for either single or married women, it just depends on your preference.
