  1. Dear Sir/Madame

    I wish to make a formal complaint regarding an item purchased in your store/restaurant/bar (name of establishment) on the 12th August 2008.

    The item in question was an onion bhaji, the complaint being there was a hair inside which as you can imagine has caused me considerable distress and concern for a number of reasons but most importantly the health and safety issues surrounding the food preparation and hygiene regulations in your establishment.

    I am treating this matter with great urgency owing to the nature of my complaint and I would appreciate the matter being looked into straight away and the correct procedures followed.

    I trust that you will treat this matter with the utmost importance and I look forward to hearing from you within the next 5 days, otherwise I will have no alternative but to report you to Health and Safety.

  2. You don't write a letter...You call the manager over and SHOW IT TO THEM!  and then you never eat there again.  They will go out of business soon enough.
