a) It all depends on your state laws, 18 to go by yourself, otherwise 16 with parental consent

b) It depends on how well you tolerate pain, for me personally it didnt hurt, it is like getting your ears pierced, yeah it hurts but only for a moment.

c) Different places have different prices, you dont always want to go cheap though when it comes to needles and your body.

d) If you dont think you can take care of it i wouldnt get because although oral piercings heal quickly they do get infected easily. They will suggest an mouth wash for you to use. If you are a smoker you should be sure to cut back on the cigarettes and rinse you mouth after smoking, if you are unable to do so then wait to have a cigarette.  Also you have to rise when you wake up and before you go to sleep and after each meal.

Normally when you get a piercing at parlor they will give you a pamphlet specifically directed toward the type of piercing you have (EX: oral, surface, naval, etc.) and this will provide you with the information you need to know about cleaning, healing time, and things to avoid.
