Can anyone recommend a book or web site that can help me write Long term Care policies and Procedures?

You want a web site to help you write an insurance policy.

Honey, it doesn't work that way.

Start with the library at your state insurance commissioner's office.  You should be able to read through it in about two years.  

Any policy you write, needs to be compliant with state law.   If you don't want to hire lawyers, underwriters, and actuaries to do this, you're going to have to read enough to be a lawyer, underwriter, and actuary.

But feel free to publish your work  for free, after you've done it all, so your competition can copy it off your website for free and put you out of business.

You forgot to mention that there are also Federal laws and regulations regarding insurance companies as well as tax ramifications. I think a good place to start might be with the IRS code. It will probably be easier to understand than the Department of Insurance info:)
