Looking for information on piriformis and nerve entrapment release surgery?

I am a 35 year old male in decent physical shape, and I have been suffering from piriformis syndrome for almost a year and a half, and a recent neurography has revealed entrapment of the sciatic nerve at the piriformis level and near the quadratus femoris. I have tried virtually everything -- physical therapy, stretching, acupuncure, massages, meds, etc. I am under heavy pain meds which don't even do much to relieve the condition. The pain has literally taken over my life, as I am unable to sit for very long, cannot focus or exercise, etc. I recently saw a neurosurgeon, who after unsuccessfully injecting cortisone, botox and anti-scarring medications into the muscles, would like to perform surgery to release the sciatic nerve at the piriformis level and lower down buttock at the the level of the quadratus femoris. Has anyone out there had this surgery and if so what was the outcome, side effects, recovery time, etc? Thank you!
