Lost Jury Summons Form?

I have Jury Duty starting on Monday and I have lost my Summons form.

All the information says I most take this form with me. This could pose a problem. Has anyone else had this? What do they need the summons form for? could I provide the information they need in a different form?

  1. it`s just to make sure you are who you say you are, just turn up and they will have your details on file anyway.


  2. Just go and tell them you lost it.  Believe me.  You are in their computer.  I know, I always lose mine, but they still have me listed.  It is not a problem.  And don't fail to show up just because you lost it.  That WOULD BE A PROBLEM.

  3. Old Know it All has it perfectly correct in all forms!

  4. They'd soon find you if you didn't show up.  Go to court and tell the clerk that you have been summoned but you've lost your form.  He'll sigh a lot, mumble, make you wait, then ask you for all the information he needs to fill in his form instead of copying it from the summons.
