It's at the office of records for the place of birth.  This is usually at a state's office of records, but it could be at the county or city records office depending on the state you were born in.

Usually the county you were born in keeps a copy. Call the courthouse in the county where you were born and order a certified copy of one.

Assuming you know the name of the town where you were born, just contact the office of the County Clerk or the Office of Vital Statistics for the City and request a new one.
