MIDDLE NAME as Given name or Last name on change of name form?

In the change of name form, there is a space for Given name(s) and Surname, where do I put the applicants middle name out of these two?

For eg. Given name: John

Surname: Williams

Where do i put Keith (middle name)?

  1. You put it after you put John. It's one of his names so you put it there.

    It should look like

    Name(s) John Keith

    Surname Williams

    There you go!

    Given name(s): John Keith

    Surname: Williams
