  1. the blocking creature remains damaged

  2. The blocking creature remains damaged until your turn ends. it "Heals" when your  turn ends, but between the combat step and the end of your turn, the damage from combat (and other things such as direct damage) is still there. So if you blocked a Silver Myrr (a 1/1) with your Steel Wall (a 0/4) the Steel Wall wouldn't die. but if your opponent plays a Lightning Bolt ( 3 direct damage) on your Steel Wall, the total turn damage would be 4, Steel Wall's toughness. Steel Wall would be sent to the graveyard.

  3. As the combat phase ends and last main phase is over, your creatures (surviving creatures) then recover from any damage that occurred.  However, the "wither" damage is permanent and stays that way unless you remove the counters  or return the creature to your hand.

  4. All damage remains on the creature until the end of turn.  At the end of turn phase, there is a damage clean-up step where all damage during that turn is removed.  Counters remain on creatures, so +1/+1 counters and -1/-1 counters will last until the creature or the counters leave play.  This is important because of power/toughness modifiers and direct damage.  If a 3/3 was dealt 1 damage, you can shrink the creature down to have 1 toughness and then the creature will die because of the 1 damage that's already on it.  Also, if the 3/3 creature is Shocked after already receiving 1 damage, it will die because it will hve 3 total damage during the same turn.
