i need help on making a personal survey/ test type thing that just ask for things like favorite this favorite that, whats the most embaressing thing your parents wear, what do you plan to do for a career

( things about someones personal interest)

this is for my dance team to get to know each other better

and since i am birthday planner for my team i need to know what kind of things i can decorate their locker with on their birthday

and if you have any ideas besides decorating lockers

feel free to put them down

but it has to be an idea towards birthday themes
