Making phone calls to Japan from America?

How do you make a phone call to Okayama, Japan from CA? Do you need a phone card? If you just call without phone card, do you need any international access code? Do you need any area code? Basically, please give me an example with a fake phone number in Japan and how to call it. Thank you
