Your patient is well educated, married, a 41 year old professional with 3 children, a successful business, well-known participant in local charities who insists that her life is in order. You have been diligently searching for a rational explanation for her complaints: poor appetite, diarrhea on occasion, mood swings, recurrent upper respiratory infections, nervousness, myalgias and joint pain.

You have run every test available and they are all negative. What is your diagnosis???

Thanks for any help!

  1. somataform/conversion disorder - ref to psyc

    dd = munchousins

  2. Sounds like my typical day - she's fine.

    By "every test available" I hope that means a GI study, because it could be stress-induced colitis or irritiable bowel syndrome.

    I would say it is Lupus.

  3. Every test meaning you've ruled out autoimmune disorders like lupus or rheumatoid arthritis or ulcerative colitis....

    Anyway, if ALL ELSE is ruled out, and I mean EVERY possible disorder that could affect any system mentioned (neuro, GI, immune, endocrine, etc.) then diagnose with IBS based on the GI complaints. I would also maybe diagnose her with a generalized anxiety disorder, especially if it was found that the symptoms were exacerbated by times of stress.

    That's the best I got.
