Medical forms or vaccinations needed for green card app?

This is for my green card app through family-based category. Which medical forms or vaccinations are needed? Is there an expiry on the signed form? thanks.

You will have to prove that you have had the following vaccinations:

Measles, Mumps, Rubella, Chicken Pox, Polio,

You will have to test negative for the following:

TB, HIV, Syphilis

They will also ask you if you suffer any of the following:

Alcholism, Gonorrhea, Mental Defects, Drug Addication, or Insanity (I swear this is all on the form!)

I believe that the immigration physial is good for 6 months (not sure on that though) you should have it the week before you intend to file as it takes 3-5 days to get some of the test reults back.
