Memory card, card reader, formatting?

Question

I just buy a phone with memory card and card reader given. First, I put music files in it,then i format it. All the files have vanished. then, I put my memory card in the phone. The phone says 'Insert Storage Card' when I want to open my media files. how to make my memory card to be back normal again

just format it or have your card reader fixed, this happened to me and i had it fixed, its a bit annoying
