Question:

Methadone and Ultram?

Would a pain management doctor prescribe these two together.

Keep in mind that she prescribed me Paxil and Ultram together, but I have not started the Paxil yet.These are known to have a major drug interacton.

Also keep in mind that she put me on Fentynal 12mcg when I have not been taking opiate pain medication on a regular basis and no one has tried to treat my pain with oral pain medication.

I'm no longer on the Fentynal because of an allergic reaction to the Fentynal.

So do you think she will put me on Methadone with the Ultram even though Methadone has opiate blockers?

Unlikely...but I'm not psychic (not that anybody is, but you didn't ask)  Obviously there is a lot you are not telling us, such as why you have pain, and why antidepressants have been indicated.   You should discuss all this with your pain specialist if you have concerns about medication side effects.  That is their area of expertise, after all.

    Ultram is a mild opioid..similar to codeine.

    Fentanyl patches and Methadone are pretty much the pinnacle of opioid potency,



    I find it very unlikely that you would be put on either Fentanyl or Methadone as a first line, without going through all the other analgesics first.

    The AltMed room is probably not the best place to be asking questions about pharmacology and medication interactions, as very few of the AM crowd have any knowledge or training.

    EDIT:  Fibromyalgia rarely benefits from opioid analgesics.  The cause of pain in FM is not clearly understood, but is likely related to changes in how the brain perceives normal nerve stimuli, and mis-interprets them as painful.  That is why some of the neurological drugs or antidepressant drugs usually work better than analgesics.  Also, there is a very limited role for medication generally in FM.  Non-pharmacologic options are preferred, such as exercise, counselling,  good sleep and nutrition, relaxation techniques etc.


  2. if I were you I would refuse paxil.  it has major side effects like suicide and depression and can really mess you up.  Do some research on these meds before you accept them.

  3. YOU are in control of your healthcare. If you prefer one thing over another, tell them! I wouldnt be mixing drugs when you know youre not supposed to..Ive heard from alot of people that Paxil doesnt work anyhow...But its good that you have a doctor who atleast knows youre in pain, I see a pain management doctor too, and I have to BEG him for percocets! But yes, youre paying your doctor...so tell them what works for you, or go see someone else who will listen to you.
