Middle name-maiden name?

Question

my middle name is louise but on forms my mom keeps on putting my middle name with my first name and on the middle name box her maiden name.so its like ths:

First name:Michelle JoAnn Middle:Johnson

my mom ticks me off cause when i graduated 8 grade on the program they put everyone's middle name and the school put mine as JoAnn.

Please help mom's drivin me nuts
