  1. The best way to get to the Duomo is to take the train from the airport to the Cadorna Station in Milan. The Malpensa Express train runs every 30 minutes between the airport and Milan's Cadorna Station. The travel time is 40 minutes at a cost of 11 euro each way. You can buy a day ticket that will cover both the trip to Milan and back to the airport for a cheaper price than two one-way tickets.

When you get to the Cadorna station, take the Metro or walk to the Duomo. If you take the Metro, it's 3 stops on the red line; there's a map of the metro system available from the link below.

There's further information here (train schedules, a link to the metro site, ...):

Make sure you leave in time to get back to the airport and get through security for your flight.

There are also buses from the airport to Milan, but they take 50 minutes or more (with traffic) and end up further from the Duomo. This is the option to take to get to Milano Centrale station, but not for the Duomo.

A taxi will run about 90 euro and will not be faster than the train.

  2. you take the train from the airport and you arrive to the final station of Cadorna...

then it is 10 minutes on food if you take via Carducci and corso Magenta...

the cost is 11 euros, and it takes about 40 minutes to get there..There is train every 30 minutes
