Not sure exactly, but most likely they purchase an address list from area jr. and sr. high schools.  This is the same way photographers get your info for senior pictures.

Yes you have to pay to compete.  It's a 'registration fee.'  Usually girls get sponsors (family members chip in, local businesses and/or neighbors) to pay this fee.  Often there will be a pageant program where the sponsors are listed.

I competed in the Miss Teen Michigan All American when I was 15.  It was fun.  If you're wondering if it's a legitimate pageant, it may interest you to know that Halle Barry was the Miss Teen Ohio All American, then went on to win the national title.  The next year she was Miss Ohio in the Miss America pageant, and took first runner up.
