  1. If an employer elects a premium plan that varies by employee types (union, salary, etc), they do not have to pay the same percentage. However, depending on the type of plan that it is (full indemnity, self-insured, etc), the employer can create a lot of headaches for themselves if they favor one group over another.

  3. No.  Employers can ALSO pay a flat dollar amount towards premium - like $100, or $200.  

    Employers must pay the same percentage or flat dollar amount, for all employees in a particular class.  So, they might pay 25% for all hourly employees, and $300 for all salaried employees.  

    Or, $300 for single employees, and 50% for employees on the family plan.
