My 41/2 year old daughter is peeing & pooping in pants most days & still bed wetting any ideas to help?

My daughter has been potty trained for over a year now but for the last 3 weeks she has been poopoing & peeing her pants at daycare, but not at home nor at my parents when she goes for her Saturday sleepover.  I have questioned her about the daycare which she has been going since she turned 1.  I have questioned the daycare lady and says nothing has changed there in the house.  I am so frusterated because she knows and tells me what I want to hear.."Mommy, I will go pee & p**p on the big girl toilet"  But she says one thing and does another.  I asked the daycare if she is yelling at her and maybe staring her from going and she says no, she is tiotally calm with her.  I am very short fused with this because she knows, but continue to do it..Is this just a phase?  She is a only child and I had her when I was 40 and as we all know the older we get the shorter our patience are.  Can anyone give me any ideas on how to break this.  The sticker systme does not work.  I have taken her computer away, her treats, some toys and she gets upset when it happens and then just carry's on like no big deal...HELP
