My Grandma keeps asking me to shave her back, how do I tell her that I don't want to?

Tell her it is not in your job description as a grandchild to perform this task and buy her a spa treatment where they can wax her back for her. ( it would be in their job description)

She should not be having this shaved anyway waxing or electrolysis would be a better way.

wha?...o-keyy...look at it this way, when you're shaving her back, keep the clippings and knit her a cute little sweater. that's a way of saying thanks for everything she's done for you. jk
