My birth certificate is messed up?

They have my Mom's first married name as her maiden name, does anybody know how I can get this fixed without seeing a lawyer? My son is serving in Iraq and I need to get a passport just in case of a emergency. Thanks for your help.

Since it was your mother who completed the form that led to your birth certificate, it will be very difficult for you to change.  It is possible that she used her first married name as her maiden name, and that is not illegal or even incorrect in the US.  If that's so, then just fill out the passport application with the information from the birth certificate as it is.

Execute an affidavit of correct information and present this document to correct the entries on the birth certicate at the Civil Registry.
