  1. A 40,000 platelet count is pretty low.  Are you sure you don't have any "rashes" or anything?  There are a number of things that can cause a decrease in the number of circulating platelets, including idiopathic (cause unknown), gestational, but you said you're not pregnant, etc.  Were there any other abnormal blood findings, like rbc or wbc abnormalities?  Have you had a cbc done in the past?  If so, what was the platelet count then?

  2. A platelet count of 40,000 is definitely low. The most common cause of a low machine platelet count is clumping of the platelets. It leads to a low number but a high MPV or "mean platelet volume"; so look at that number on the report. If it is high then just repeat the test. Looking at the blood microscopically allows one to see if the platelets are clumped.

The causes of a true low platelet count include low production from disease of the marrow such as infection (often just a simple viral infection that clears quickly), or tumor. Low platelets also comes from consumption of platelets from bleeding (anywhere) and destruction in the spleen.

There are clues to these are also seen by looking at the blood.

There are many causes to late periods, of course, the most common is pregnancy but that is probably NOT related to the platelet count.

  3. A platelet count of 400,000 is at the upper limit of normal.

Your real problem is secondary amenorrhea.

Ask your doctor if an MRI of the pituitary is appropriate in your case. There are a lot of possible causes, but as a brain doctor I tend to think of all the pituitary problems I have seen with amenorrhea first. Here is a reference for your review:
