  1. Most of the answers gave you almost the right answer. Yes, you can download the forms from the government website BUT you must apply in person the first time you apply for a passport. The forms tell you everything you need. Go to your local post office, you can hand in your application, with your money, birth certificate and photos.

Not every town/city has a passport office but if you live in a town/city that has one of course that is a better way to apply.

Good Luck and allow 6 weeks to get your passport )

  2. Forget about going to a Post Office.  Just go to the US Passport office web site.

    http://www.travel.state.gov/passport/pas...

It has the application form that you can print at this web link:  http://travel.state.gov/passport/forms/d...

Then you will need two passport sized photos, a certified copy of your birth certificate, and the application fee which is $100.  You must each submit your applications separately with the required documents and fees.

It takes 4 to 6 weeks normal processing time to get a passport but if you need it immediately and live in one of the cities that has a processing center you can get it in 14 days or less.  The regional processing centers are listed on this web link:  http://www.travel.state.gov/passport/abo...

  3. The post office has the forms and some of them also take the required photos.

Be sure you leave plenty of time [6 weeks] to get the passports.  You can pay extra to have them expedited, however.

  4. Go to your local post office or passport center.  Here's where to find them:

    http://iafdb.travel.state.gov/

    Info on the application:

    http://travel.state.gov/passport/get/fir...

  5. I went to city hall to fill out the forms. I brought my birth cerificate, social security card, driver's lisence, etc. I also went to CVS (a drug store) to get a passport picture made.
