  1. You have a misalignment in the sprockets or the derailers which is causing the chain to run crosswise. It may be due to a bent derailer or to a warped sprocket or it may just be that the derailers need to be adjusted. Have a bike shop take a look at it.

  2. you have bent linkage...take it to the bike shop for a new part

  3. Have the bike shop check for three things:

    1) chain wear.  The wrench at the shop should use a tool called a chain wear indicator to do this: It looks a lot like a steel ruler.  A worn chain can cause the chain to skip under pedal pressure.

    2) Make sure the derailleur hanger is properly aligned.  The derailleur hanger is the part of the frame that the derailleur is attached to.  Depending on he make and quality of the bike you might have a "sacrificial" hanger which is meant to break away before the derailleur is damaged in an accident, or a traditional hanger which is part of the rear dropout.  If it's a very cheap bike, the hanger is incorporated into the derailleur itself.

    3) Check to see that the rear derailleur is adjusted and indexing properly during shifting.

These three checks should cost you little to nothing, and you'll either ride away in 15 minutes with the problem solved, or know what the problem is and what you need to spend to get it resolved.

    Good Luck!
