  1. Don't give him a diaper at four years old! But if you don't resolve this, he will start holding his #2 too long.try giving him a freestanding potty to p**p in (not the real flushing toilet) for a while. Explain to him that everyone poops and everyone poops in the potty. Tell him he doesn't have to flush, if the flushing is what makes him nervous. He does NOT remember what happened to him at 4 months old, only you do, so stop making excuses for him and let him p**p in his underwear if he refuses to p**p in the potty, then when he poops in his underpants, make him clean it up (remove the p**p underwear, etc.). Make sure his diet is such that he can go easily (lots of fiber, water, etc).

  2. What i did with my little ones is made it like a big party when my little ones went #2. We even went for ice cream...sort of made it worth their while.

  3. Make a sticker chart for him. Every time he goes p**p on the big boy toilet he gets a sticker on his chart, let him pick the sticker out and you put it on the chart. Let him know after getting 3-5 (u pick the # make it low to start with) stickers he gets to pick a "prize" from the special basket. Go to the dollar store and buy cheap toys; stickers, balls, dinosaurs, cars, bubbles, etc... make it fun for him. Usually a visual thing like a sticker chart with rewards will work for situation like this. If he is constipated start feeding him more fiber. Also you know that holding in his p**p is not healthy for him if this continues make a doc. appt for him. Be patient with him, he is scared for a reason. Remember to be positive with him, hugs, high fives, thumbs up, a wink some kind of a gesture to let him know that your proud of him for going p**p in the big boy toilet.
