  1. You all need your birth certificates and adults need a photo I.D.  If your childrens other parent will not be with you, you must have a letter of permission to take them out of the u.s. signed and notarized...from the absent parent....unless you have documented sole legal and physical custody.  The different last name in itsef is not at all uncommon and should not be a problem.
