My doctor has just signed me off work for 2 weeks, i have the signed certificate to give to employer

do i also need to get form SSP1 from my employer? CONFUSED!!!!!!

  2. I think you just sign your doctors line (it says form Med 3 on bottom right of form) and give that to your employer.  That is enough for you to get statutory sick pay (its all I had to do anyway).


  3. Check with your employer.  Most large companies will give you full pay if you are off sick without you having to claim anything.

  4. Dont sign until you have seen Living Design tell your employer to p**s off u have a sick line and they cant question that you fill in the ssp when your well mate

  5. Fill in the SSP1 when you get back to work. It covers the first few days of sickness and works in conjunction with the sick ntoe from the doc.
