Question:

My family is taking me and a friend on a cruise! We have the form signed but is there anything else?

Okay! We have got the form to take her out of the country. She has her birth certifacate, insurance card, and school id. Is that all we need. Is there any other form? or something else we need!

  1. you will need a passport very soon....check with a travel agent or the cruise line and explain what you have.  they will tell you everything you need to know

  3. You asked in the prior question if you needed a medical authorization.  That is contained in the permission letter.  I provided you with a link to the permission letter when I answered your earlier question.

    I strongly recommend getting a passport.  I don't know when your cruise is scheduled, but anyone without a passport now is in danger of denied boarding.

    Also, most likely both your friend's insurance and your family's insurance will not cover your friend or your family while you are outside the United States.  You might want to look into getting travel insurance.  Many companies offer travel insurance.  I offer the following website because it offers policies from many different carriers ...

    Finally I am a little concerned that you have to ask these questions.  A good travel agent should would be more than willing to help you with these issues and more as part of their service to you.  It is free service to you, since the travel agent gets his/her commission from cruise line.

    I have a great travel agent.  I have been on seven cruises and have four more booked, and I use a travel agent.  His name is Bob and he has been on over 90 cruises.  You can contact him at bobd at worldviewtravel dot com.  I write it that way to make it harder for an e-mail harvesting program to pick it up.  His number is 800-869-0674.  Please understand that for him to provide you with any advise you would have to be his client, which means transferring the booking to him.  Your current agent, who apparently is not providing you with much info, might charge a cancellation fee.

We took our niece on a cruise and at that time we just had a note signed by the parents in case of emergency bla bla bla, and her birth certificate but her last name was the same as my BF they did not ask or demand anything I don't even think we gave them the note we just had it with us for just in case.

    Please call your Travel Agent and or the cruise line NOW! to ask this question you can't fix anything the day of the cruise.  You must call them now! Go on.... call them now.........

  5. Yes, the form giving permission has to be signed by all parties (mom and dad of the unacompanied minor) and both parents that you are traveling with. IT HAS TO BE NOTORIZED TOO. If it isn't they arent going to let you board the ship. If the cruise ship is going anywhere other than North  America you also need a passport. Have a great time!

  6. You might think of getting a notorized letter stating that the adults have dicision making athority for medical and other major events which may occure.

  7. You will need a passport starting in January of 2008, I believe. Check with the cruise line just to make sure, though.
