It looks fluffy. I just hope it's not his innards

  1. Parasites would not be "fluffy".  I would also guess it's fungus, wich would appear "fluffy".  Luckily, you can easily treat it with most Ich medications-- anything malachite green, copper, or nitrofurazone based would be very effective.  Remove any carbon from the filter during treatment, and if using malachite green, keep the tank dark, as it degrades with exposure to light.

These are more severe fungal infections: http://www.uib.no/bot/kurs/BB220/Algae/F...

  2. It is under his scale?  Chances are it may be a parasite.  Different types of worm parasite's can burry themselves beneath the scales.

If it is large enough to see, and your fish is large enough, it can be removed by hand, however not knowing if there are larve in the tank, you should treat with salt dips.  These parasites can be from leaches, lice worms etc.  without knowing more about his everyday habits, eating not eating, lethargic it is difficult to say

  3. It Can be  funus from hurting himself try putting in some aqaurium salt
