Well I'm not too sure but I would just tell him to go to a doctor, it could just be normal but you never know and a doctor is the only person who can treat it if something is wrong.

It's a hernia, its not extremely bad but it could start causing pain if left alone too long. that happened to me, just go to your regular doctor and he will determine if it is, then he will set you up with a surgeon and he will take at it, then he will set up a date for the surgery. it's an out patient procedure.

He needs to be checked and evaluated by a doctor.   He may actually have two testicles, but a hernia could fill the s*****m and appear to be one large t******e.    There could be other conditions causing this, but urge him to see a doctor.
