My friend is scaring me greatly?

my friend is worrying me. he is holding his pee and not urinating for 24 to 40 hours. he said he wants to not start school. i wanna know how sick he could get if he kept doing this to himself. could he get cancer or a deadly infection from simply holding urine? should i intervene?

He is putting himself at risk for kidney damage, infection, or even rupture, and possibly cancer in the future.  It sounds like he is already suffering from brain damage.  Do not try to intervene, it could be contagious.  Go to school without him.

PS: Tell his parents.

How old is this guy? Not urinating can put you at risk for infection, or it can stretch your bladder so you lose the feeling of needing to go, and maybe you won't be able to control it after you do it enough. Tickle him until he wets his pants :).
