Question:

My last name is considered as my middle name on all records. How can I change that to be corrected?

I was told by my mother a long long time ago that the nurses at the hospital where I was born at made a mistake on my birth certificate by putting the wrong last name. The wrong last name appears on my driverse license, but on my social security card, it is correct with my real last name. I've been carrying the wrong last name with me for over 25 years now. Unfortunantly, I haven't had any real major problems with it, but now I am. I was told that I would have to contact the hospital I was born at, but it no longer exists.

  1. Every county keeps records of births.  Contact the registrar, county clerk, or whatever the department is for the county where you were born.  Ask them for the forms, what proof is necessary to "correct" it, etc..
