  1. In addition to filling out the form 1040, she should print "Stimulus Payment" at the top of the form. This was on the irs.gov site.


  2. Yes, unless someone has claimed her as a dependent.  

    To get her check, print out the special version of Form 1040A available at irs.gov, have her or her POA sign it and mail it in.

    You have until October 15 to get the check.

    Added later:  You indicated she lives off her check, so I assume her annual benefit is at least $3,000.

  3. She must file a form to get the stimulus check

  4. if she files and has ss income of at least $3000

  5. If her SSA-1099 shows $3000 or more in box 5 *AND* no one else could claim her as a dependent, then yes she's eligible for a $300 rebate by filing a return.

    (Use 1040A, write stimulus at the top, put the SSA amount on the SSA *A* box, sign and mail.)
