My phone got stolen and I will file aclaim with Asurion. But I think Ive filed more than 2 claims in 12 months

Question

My mother left my phone at K-mart the first time, and it was stolen.

The second time it broke really bad...but then again I'm not sure if I filed a claim or not because I remember taking it to the VZW store and they gave me a new phone for free...but I kept my original battery...

My question is if there is a fee for more than 2 claims in 12 months and how much it costs...

you probably have to pay the full retail cost of a new phone - several $100
