  1. I think this is the form you are looking for.

Although you have to file this form in US before you leave.

  2. Depending on the reasons you needed to leave for an extended period and how long you anticipate having to be away, you can file an I-131 and get a "Re Entry Permit". Yes biometrics are involved. If you are already outside the U.S. contact your nearest U.S. Embassy as many will help you appply and recieve one.

The I-131 form is found at www.uscis.gov under forms.

  3. well, you would have to file for a re-entry permit before leaving the US. so, it's a bit too late for that for now. If you remain outside of the US for over 12 months, you will almost certainly lose your status and then the only way you could regain it would be to qualify all over again. So, whoever filed a petition for you the first time would have to do so again, unless you got your green card as a dependent. In that case, you'd have to find some other way to qualify, if you can. It might be possible to qualify for a returning resident visa, if you can prove this occured for reasons beyond your control, but based on what little you've said here, that may well not apply to you. If you end up trying that, you'll need to go to the nearest US embassy or consulate and inquire.
