My teacher gave me this gift kit anyone know what it said???

Okay this gift was like a kit. It had a rubber band, a band-aid, a kiss, an eraser, and a couple other things. Each thing stood for something in this poem. I'm now a teacher and really wish I could find this poem to give to my students.
