  1. 1. An account of the event or events that led you to bring suit

    2. A detalied list of the damages you're trying to collect. Make sure you include any proof such as pictures, reciepts or police reports

    3. Any eyewitnesses that you may want to include


  2. 1. At the top center leave an underlined space for a court docket number to be stamped in.  2. At the top right put your name vs. the defendants name.  3.  On the top left put the name and number of the court.  4. First paragraph put names the parties, plaintiff and defendant 5. Second paragraph give the address for service of citation for the defendant.  6.  Third, describe the facts.  7.  Forth, describe the damages or injuries.  6. Fifth, state an amount of damages that you are suing for.  7. Sixth, Prayer.  8.  On the left side type your name, address, telephone, fax, etc. above a signature line  9.  Sign it.

    You don't include any evidence or pictures, etc.  If you want to see a few real ones, go to the courthouse and ask the clerk to show you some, they are public record.
