Need a website for completely free "consent for childs medical treatment" forms

I am leaving for vacation next week ad need a medical cosent for to give my family member for my daughter whie I am a way. I'm not paying for something I am only using once, so my question to you is, what is good, LEAGAL site that is also totally free and will give me what I need at no cost. Thank you!

My father just types up a small form that gives all the medical information (I have a heart condition so people have to know what to do in case of emergency) he has very good formatting skills so it's just a small piece of paper folded up. He puts the medical release on it, and my parents sign. It has contact information, and my doctor's information.

    Talk to her doctor about what information should go on it and type it up. Make sure everyone signs it, and put all contact information and anything specially required.

    Also make sure you make a few copies. Keep them in safe places (wallets are good) somewhere people will know where to find them but won't be lost.

  3. You don't need a specific form. This is what I have for my mom and step-dad. I work for an atty: and he drew this up for me

    The undersigned, YOUR NAME HERE parents of CHILD'S NAME HERE, hereby grant and convey authority to FAMILY MEMBERS NAME HERE to act in loco parentis (underline loco parentis) as though they were parents of CHILD'S NAME HERE.

    We grant this authority on DAYS HERE (ie 8-11-08 at 9:00 am to 8-15-08 at 5:00 pm)

    This authority is given so that FAMILY MEMBERS NAME can make any and all medical decisions as well as decisions for health and welfare for  CHILD'S NAME as set forth above.

    DATE:      than                            Your signature.

    I would also include your insurance information and give them a copy (front and back) of your insurance card, if any. Wouldn't be a bad idea to have it notarized.

Are you sure you need a form?   I simply typed up my letter and had it notarized....many, many places do not charge for notarizing items (many banks).  "To whom it may concern, I give Suzy Jones permission to act on my behalf to obtain medical care and make medical decisions for Sam Smith in my absence".....something like that.   Be VERY specific.
