  1. > cleft pallet of the soft pallet

    This can be surgically corrected.

    > extra piece of chromosome in 1/4 of her cells

If it's in only 1/4 of her cells and not ALL of her cells, then it's a problem that occurred during development.  This isn't likely to happen again if you get pregnant again, since it's not from your chromosomes or your husband's.

Precautions you should take would be to stay away from mutagenic chemicals.  During your next pregnancy, stay away from benzene, don't get gasoline on your hands (and try not to inhale it), avoid using certain bug sprays.  And, of course, avoid all medications which have the warning "pregnant women should not handle..."

  2. Well, since your daughter is the only person in the family to have the condition of cleft pallet, then you and people in your family are carriers on the condition without showing signs of the disease. So it appears that because nobody else in your family shows signs the there is VERY little chance for the condition to happen again. It also appears that the condition is only shown every few generations... Which means that one or more of your ancestors had the condition.

  3. I would suggest you talk with a licensed genetics counsellor. The presence of an additional chromosome suggests a potentially more serious condition
