Need help in filling out the EMA form, bank details !?

I am just appling for the education allowance but I got stuck on the bank details.

It says Name of Bank/Building Society and Branch ... so should I put for both of them HSBC ? or not ?

Also where can I find my sort code on my card ??

  1. For the name of the bank, put HSBC, for the branch, put which town/city/village/whatever you opened the account in.

    I found my sort code along with my account number on one of the first letters my bank sent me when I first opened my account.

  2. The details you will require are

    Bank/Building Society Name: HSBC BANK PLC

    Branch: Will be the town you opened it for example manchester city centre

    Address: Look on your statement or chequebook

    Sort code: will be the number on your card that looks like 40-16-28

    Account number  = your 8 digit card number
