Need help w/ writing "intent to vacate" letter to landlord?

I asked this same question earlier, and based on responses from Yahoo Answers viewers, I have revised my "intent to vacate" letter to my landlord. I appreciate all responses, but am again asking for help with this letter as well.

In accordance with our month-to-month rental agreement, please find this letter as my formal 30 days written notice to vacate the premises at (apartment address).

Persuant to Ohio Revised Code 5321.16, I would greatly appreciate the return of my security deposit of $450 within 30 days of vacating the premises. Please forward the return of my security deposit to the following address:

  1. The ONLY other idea that I would have is to give the exact date that you will vacate the premises .....

    Also; upon vacating ; take pictures .. lots of pictures so you can prove there was no damage .....

    Note: Rent continues until you have given the keys back to the landlord .
